Chronic cardiovascular sequelae Post-SARS-CoV-2

Introduction. SARS-CoV-2 infection represents a public health problem worldwide, even several years after its appearance, the development of chronic pathologies shows a series of complications at systemic level, with main affectation at cardiovascular level in a chronic way, this has led to the investigation of the pathophysiology and the consequences that may occur in the presence of this pathology. Objective. To determine the impact of chronic cardiovascular sequelae following SARS-CoV-2 infection. Methodology. A systematic and critical search of scientific journal articles was conducted, analyzing the publications on cardiovascular complications caused by SARS-CoV-2. For the bibliographic review, 126 citations were analyzed and subjected to inclusion and exclusion criteria, resulting in fifty-four articles selected for the present review. Results. SARS-CoV-2 infection chronically affects the cardiovascular system through immune system activity, cytokine activation, endothelial and myocardial damage, causing structural and functional modifications; the pathologies most frequently mentioned in the studies analyzed are heart failure and arrhythmias, cardiomyopathies, vascular ischemia, postural paroxysmal tachycardia, and high blood pressure. Conclusion. Chronic cardiovascular sequelae following SARS-CoV-2 infection have a significant impact on the population due to the increase in short- and long-term morbidity and mortality rates, as well as a decrease in the quality of life in apparently healthy patients and in those with pre-existing comorbidities.

Case report of a canine patient with hypoadrenocorticism treated with fludrocortisone at the "Argos Veterinary Hospital"

Hypoadrenocorticism is an underdiagnosed or underdiagnosed disease in small species veterinary medicine in Ecuador. The objective of this study is to report the treatment and diagnosis of this pathology to identify it in initial stages and to administer a correct therapy. The patient came to the clinic with a nonspecific condition, the guardians reported that she was shivering, had no appetite and had decreased her physical activity 3 days ago. In another service she was prescribed omeprazole at 1mg/kg and meloxicam at 0.05mg/kg. The medical team of the Argos Hospital identifies in the physical evaluation moist and pink mucous membranes, CRT 4 seconds, RR: panting, severe cranial abdominal pain. Normal state of consciousness, weak pulse, SpO2: 93%, systolic pressure: 145mmHg. General examinations were suggested at the Argos Veterinary Hospital, hemogram, blood chemistry, coproparasite, abdominal ultrasound and pancreatic lipase. The results showed moderate neutrophilia, hyponatremia, and hyperkalemia, with positive pancreatic lipase (snap IDEXX). The picture is evaluated together with the Endocrinovet service, and a possible hypoadrenocorticism is determined. A basal cortisol measurement is indicated, which results. Evaluation of the effectiveness of an administrative intervention on the preparation processes of Mueller Hinton agar in two public hospitals in Ecuador

Introduction: Mueller Hinton agar is one of the most used media in microbiology laboratories to perform the antimicrobial susceptibility test, since it has a particularly good reproducibility, it has low levels of inhibitors that can affect the results of sulfonamides, trimethoprim and tetracyclines, allowing the growth of all non-fastidious bacteria. The objective of quality control is to reduce the scarcity of the results obtained in the sensitivity tests, because of the different concentrations of the divalent cations Mg2+ and Ca2+, which can alter the sensitivity towards aminoglycosides and tetracyclines. And the concentration of thymine and thymidine, in between, affects sensitivity to sulfonamides and trimethoprim. Quality control is conducted batch by batch prepared in microbiology laboratories, using ATCC-type reference strains, together with different sensidiscs. Objectives: The objective of this research was to improve the technical characteristics of Mueller Hinton agar, the same one universally recommended for antimicrobial susceptibility tests, against the ATCC strains Enterococcus faecalis 29212, Pseudomonas aeruginosa 27853 at the General Hospital of the Armed Forces of Ecuador. and at the Pablo Arturo Suárez Hospital, implementing a training process on the preparation and control of batches of the agar, since this medium requires meticulous preparation, since minimal variations in its composition cause alterations in the test results. tests. Methodology: A non-experimental operational study, of pre and post evaluation, was conducted to implement a training process in the preparation and control of batches of Mueller Hinton agar to improve the technical characteristics and performance of the agar against ATCC strains of control. A pre- and post-training knowledge assessment was conducted. Results: The degree of compliance with the conditions of infrastructure, inputs, and materials to produce Mueller Hinton agar in the laboratories of two hospitals in the city of Quito was established. In turn, the degree of knowledge about the realization of the Mueller Hinton agar and the impact of an educational intervention on them were evaluated. The degree of compliance with the infrastructure, inputs, and material requirements of each intervened health home was 64% (16/24) and 68% (17/24). The knowledge on the subject, after the training, improved in the participants. Conclusions: When carrying out a situational diagnosis of the infrastructure, supplies, and materials, it was determined that the absence, misuse or disuse, constitute the main factors that affected the development of the environment; After the training, it was possible to improve the technical characteristics of the medium for quality control with the strains ATCC Enterococcus faecalis ATCC 29212, Pseudomonas aeruginosa ATCC 27853. On the other hand, it was observed that in the absence of updating the processes in the preparation of the culture medium, is related to errors that affect the technical characteristics and their performance in antimicrobial susceptibility tests.

Reactive compensation optimization in electrical systems by the CRITIC method

Introduction. In AC electric systems, the injection of reactive power directly influences the improvement of electrical parameters such as power factor, voltages, loose reduction, line chargeability, etc. In electrical power systems, reactive compensation optimization studies are carried out using various heuristic methods that are characterized by their programming complexity and by a large component of human judgment, which greatly influences the evaluation of the results found.  Objective.  Analyze the results of the application of the Critic method to the optimization of the reactive compensation of the sub-transmission system of the Riobamba Electric Company. Methodology.  Initially, an algorithm to obtain the population of scenarios to be evaluated and the evaluation criteria is proposed, finally the statistical analysis is implemented using the Critic method for multicriteria decision making. The proposed algorithms and the subsequent statistical evaluation were carried out in the EERSA 69kV sub transmission system. Results.  From the results of the application of the proposed methods to the EERSA network, the improvement of the voltage profiles is achieved while the system losses are reduced, all this with the minimum requirements of reactive power injected into the system.

Psych pedagogical training needs of teachers in attention to the inclusive educational system, of adolescents who attend the unified general baccalaureate

Introduction: The debates on the epistemological foundations of the integration of psychology and pedagogy in the various fields of education are initially expressed in different scientific contexts based on the needs and concerns of professionals, with the interest being to deepen aspects of their educational practice. Objective:  Analyze the importance of teacher training with a perspective on educational inclusion. Theoretical foundation of teacher training needs. Identify the factors that affect educational inclusion and attention to the diversity of students. Reflect on the challenges faced by the teacher in the inclusive education system. Methodology:  used in this study is of the research-action type, it simultaneously generates knowledge and changes, connecting the cognitive aspect with knowledge and the acquisition of knowledge through the collection of reliable and accurate information. Results:  The current training system has the centrality of the teacher as a technical and irreplaceable educational resource. In addition, there is a need to continue teacher training to achieve the development of critical thinking skills in the field of education with a psycho-pedagogical approach. Conclusion:  Teacher training must be improved according to the needs and demands of a new type of education, because education is the cause of today's society and the engine of the future. From this analysis, it was highlighted that the concept of teacher training promotes the necessary preparation and orientation based on needs for the management of the educational process in its fields of action: in the institution, the family, and the community. Child and adolescent labour: A socio-legal approach in Ecuador

Child and adolescent labour are considered a social phenomenon, whose activity limits minors in their natural development, and therefore prevents the evolution of their intellectual capacities, potentials, cognitive abilities, and desires, which are essential to form a progress of integral development. The objective of this study was to analyze child and adolescent labour from a social and legal perspective in Ecuador. The methodology applied was based on the review of scientific literature and the study of the national legal framework and international norms, with a rational epistemological approach, and under a qualitative design, as it was based on written evidence oriented towards the description of the social phenomenon under study, with the aim of understanding and explaining its characteristics, through the application of analytical, inductive and deductive methods. The Constitutional norm contemplates the successive eradication of child labour, at the same time Ecuador is subject to international conventions and treaties that prohibit any type of labour-related activity, in order to protect the rights of children and adolescents, However, this is a phenomenon within the social system that needs to be treated as a priority by the State and international organisations, since, even when labour activity is accepted for adolescents from 15 years of age, other rights such as education and recreation must be guaranteed, as well as preventing labour exploitation of this vulnerable class.
