Pulp regenerative therapy in permanent teeth with closed apex using I-PRF and A-PRF as biological scaffolds

Abstract

Introduction: Pulp regenerative therapy is an innovative alternative whose main objective is to restore pulp vitality, even in teeth with a closed apex. Using biological scaffolds such as advanced platelet-rich fibrin (A-PRF) and injectable platelet-rich fibrin (i-PRF), it is possible to naturally stimulate healing and regeneration processes, favoring not only structural repair, but also the functional restoration of damaged pulp tissue. Case presentation: We report the case of a 27-year-old male patient diagnosed with pulp necrosis and asymptomatic apical periodontitis in tooth 25, which was treated with regenerative therapy. The therapeutic approach included progressive disinfection of the root canal with calcium hydroxide and triantibiotic paste. Subsequently, apical bleeding was induced, i-PRF was injected, and an A-PRF membrane obtained from autologous blood was placed to promote tissue regeneration. Finally, the canal was sealed with a bioceramic material. General Objective: To describe a clinical case of the combined use of advanced platelet-rich fibrin (APRF) and injectable platelet-rich fibrin (i-PRF) as biological scaffolds in the regenerative therapy of a permanent tooth with pulp necrosis and a closed apex, highlighting its potential as a biological alternative for pulp and periapical repair. Specific Objectives: 1. To analyze the clinical effectiveness of a pulp regenerative therapy without the use of gutta-percha, assessing whether it is a viable option within biological procedures in endodontics. 2. To compare the results obtained in the present clinical case with other reports documented in the literature where conventional pulp therapy and pulp regenerative therapy were used.3. To determine the clinical and radiographic changes observed after the application of A-PRF and i-PRF as biological scaffolds in a tooth with complete root development. Conclusions: Pulp regenerative therapy using a combined approach with platelet concentrates has proven to be a viable biological alternative for the treatment of permanent teeth with pulp necrosis and closed apices. In the clinical case presented, this approach promoted healing and tissue regeneration processes, demonstrating favorable clinical outcomes and radiographic changes compatible with periapical repair.
