Abstract

Introduction: regenerative endodontics has incorporated biomaterials such as platelet-rich fibrin (PRF) and its injectable variant (i-PRF), which function as bioactive scaffolds that promote the regeneration of the dentin–pulp complex through the sustained release of growth factors. Objective: To evaluate the clinical and radiographic efficacy of PRF and i-PRF in endodontic revascularization and revitalization procedures.

Methodology: a narrative review was conducted, covering studies published between 2020 and 2025 in recognized scientific databases. Clinical trials, systematic reviews, and meta-analyses in English and Spanish were included, analyzing clinical and radiographic outcomes in permanent teeth. Results: PRF and i-PRF showed favorable results in periapical healing, apical closure, and postoperative pain reduction. i-PRF stood out for its improved clinical handling, greater penetration capacity, and higher release of growth factors. However, some studies did not show significant differences compared with the blood clot. Conclusion: both biomaterials are effective in regenerative endodontics, with the potential advantages of i-PRF. Standardized studies and long-term follow-up are required. General area of study: Dentistry. Specific area of study: Endodontics. Type of study: Literature review.
